The Manager Adult Day Services provides supervision and oversight of Day Services Supervisors.
The Manager supports program growth and development of the Day Services program across the Agency.
The Manager facilitates the referrals and opening of individuals to the appropriate service.
Assist in establishing, developing, implementing, and evaluating individualized services plans for day services.
Completes and participates in person-centered quarterly and annual reviews.
Coordinates day services to ensure compliance with applicable Agency, federal, state, and local regulatory standards.
Provides supervision of implemented plans, observes and assess staff performance, aligns with state standards, and delivers services as needed.
ESSENTIAL FUNCTIONS :
Coordinate the recruitment and hiring of Day Services staff.
Responsible for providing training, in accordance with state mandates as well as continued observation and evaluation of standards.
Provide supervision, support, and evaluation of staff.
Ensure staff meets Agency and state training requirements Provide lead supervision to assigned staff ensuring effective and efficient delivery of services and opportunities for professional development.
Coordinate and facilitate monthly trainings, supervision, and support for staff as needed.
Meet with community partners and stakeholders to establish relationships in order to find, access, and develop community programs and opportunities for services and supports for individuals served.
Community relations should include seeking funding sources to continue to grow and access varying community-based opportunities for individuals served.
Develop, implement, and monitor individualized assessments, person-centered plans, data collection methods, and progress reports that lead to personal growth for designated individuals served, and comply with applicable funding sources and regulatory standards.
Participate in Person Centered Reviews and annual team meetings as organized by support coordinators/case managers and discuss progress, review needs/concerns, interests, and strengths of individuals and families to ensure appropriate assignment of services and staff.
Manage Day Services in accordance with the Agency mission, goals, policies and procedures, budget, and applicable regulatory standards to include:
(a) processing of referrals (b) monitoring the delivery of service units in order to ensure maximization of available funding (c) maintain/coordinate positive working relationships with customers including persons served, family members, referral/funding agents, etc.
(d) provide for audits of case records for quality assurance and (e) ensure the provision of facility and community-based options.
Provide skill building and support to individuals served to promote maximum independence and facilitate participation in community and/or facility based activities in accordance with the individual's expressed preferences and choices.
As outlined in the individual's person-centered plan, provide training, skill building, and support to include personal care, socialization, community integration/participation, volunteer opportunities, and other daily living skills.
Ensure the provisions of quality management system are followed in the delivery of Day Support services, specifically, an effective program evaluation system complete with measurable outcomes and a reporting system.
Ensure all requirements are maintained and monitored in WAMs, including supporting in identifying and addressing any service authorization concerns and minimizing the rate of PENDS and time to address such issues.
Actively participate in meetings held within the Agency, individual's treatment team meetings, and regional meetings by DBHDS as needed.
Participate as a team member on Agency committees and work groups as requested.
OTHER DUTIES :
Design and implement methods to evaluate effectiveness of program and services provided.
Provide on-site supervision to staff monthly, or more frequently as needed.
Secure physician's order and oversee the administration and documentation of medications to designated individuals.
Provide transportation for day services individuals as needed.
Ensure all required trainings are completed in compliance with Agency policy.
Responsible to maintain current certification in First Aid/CPR/AED and other trainings as required or assigned.
Perform other duties as assigned by the supervisor.
QUALIFICATIONS :
Oral and written communication skills Skills in data collection and interview Knowledge of intellectual and development disabilities, and the ID/DD Waivers Knowledge of treatment modalities and intervention techniques, such as behavior management, independent living skills, family education, crisis intervention, discharge planning, and service coordination Knowledge of different types of assessments, including functional assessment, and their uses in service planning Knowledge of formulating, writing, and implementing individualized service plans to promote goal attainment Knowledge of coordination of the provision of services by diverse public and private providers Work as a team member, maintaining effective inter- and intra-agency working relationships Demonstrate a positive regard for individuals and their families Previous supervisory experience preferred Bachelor's degree in a human services field required; QIDP/QDDP required.
Three years' of related supervisory/managerial experience in program(s) serving individuals who have intellectual disabilities or other developmental disabilities preferred.
Valid Driver's License with a safe driving record U.
